A physical or electronic signature of an individual authorized to act on behalf of the copyright owner whose exclusive rights have allegedly been infringed.
A description of the copyrighted work that you claim has been infringed, or if multiple copyrighted works are affected, a representative list of those works.
Identification of the material that is allegedly infringing or the subject of infringing activity, along with sufficient details to enable us to locate it. Providing URLs in the body of the email is the most effective way to help us identify the content.
Contact details for the complaining party, including a mailing address, telephone number, and, if available, an email address.
A statement affirming that the complainant has a good faith belief that the use of the material in question is not authorized by the copyright owner, their agent, or the law.
A declaration that the information provided in the notification is accurate, and under penalty of perjury, that the complainant is authorized to act on behalf of the copyright owner.
